• Complete name, address and phone number
• School planning to attend, listing date and year to start
• Degree sought or course of study
• Previous high school, college experience, etc.
• How this scholarship will aid you in realizing your educational goals and service to the Lord
• Financial aid office (include counselor, address, phone number)
• Any other pertinent information in assessing your application
